My current loop (It changes every week or so)

1. Run the app. Discover issues. Have an agent diagnose and add issues to issues.md.
2. Have agent fix issues.md.
3. Have agent audit the fixes.
4. Have agent repair what it did wrong.
5. Go to 1.

Or something roughly like that.
